Baluchonnage request and destination form

A duly signed request is essential to open a file. It must be sent by e-mail only: formulaire@baluchonrepit.com

We invite you to read the preliminary information section.

Please note that we will send the status of the request for each baluchonnage request within 5 to 10 business days of receipt.

Please send us your request for baluchonnage no earlier than 3 months before the start of the requested period. For example, for a baluchonnage in July, the request must be sent to us no earlier than April 1.

All applications must be sent by e-mail to Baluchon Long-term Respite Care’s head office in Montréal. The application will be received, analyzed, processed and assigned to a respite worker.
